The patient and family are the center of care. Your goals, preferences, routines, beliefs, and concerns guide the plan.

The caregiver role

Hospice provides intermittent care and 24/7 support, but does not replace the need for a caregiver plan. If care needs increase, our social worker can help your family explore resources and safe care options.

When to Call Hospice

Call (605) 389-3141 for:

  • New or worsening pain
  • Trouble breathing
  • Agitation, anxiety, confusion, or restlessness
  • Falls or injury
  • Medication questions or refills
  • Fever, nausea, vomiting, or constipation
  • Swallowing changes
  • Equipment or oxygen concerns
  • Signs that death may be near
  • Any concern where the family is unsure what to do
The Final Journey

A gentle look at what families often see

Common signs may include increasing sleep, withdrawal, reduced appetite, changes in breathing, confusion, restlessness, reduced urine output, and cooler hands or feet. Not every person has every sign. The hospice team will guide the family with what to expect and what to do.

When Death Occurs

A simple list to follow

  • 1Do not call 911 for an expected hospice death unless there is a public safety emergency.

  • 3There is no need to rush.
  • 4Hospice will guide the next steps.
  • 5Hospice can coordinate pronouncement, physician notification, funeral home contact guidance, medication disposal guidance, and bereavement support.
Bereavement Support

We're here after, too

Bereavement services are available to family members and loved ones for at least 13 months after death. Support may include calls, written resources, visits, community referrals, and grief guidance.
